Confidentiality
Southwest Trauma Informed Integrated Care ( STIIC) strives to maintain patient confidentiality. STIIC will try to ensure the privacy of patients' protected health information within the practice. Protected health information will only be accessible to the provider within the practice and any ancillary persons whose administrative services may be utilized by the practice. STIIC will not share information with third parties outside the practice, except in exceptional circumstances whereby a provider reasonably believes that the patient is in imminent danger of harm to themselves or others or a vulnerable person is being abused or neglected, or the law requires certain information to be disclosed. STIIC will In addition only release protective information if a release of information is signed by the patient. , To ensure safe coordination and collaboration of care limited information may be disclosed to pharmacies, laboratories and other providers of ancillary health care services. Due to the nature of billing, the name Southwest Trauma Informed Integrated Care or similar may appear on a statement from the payment card issuer.
Security of Records
Southwest Trauma Informed Integrated Care ( STIIC) maintains the security of patients' protected health information within the practice. STIIC may utilize an electronic health record and telecommunications vendors to conduct services. STIIC will maintain a business associate agreement with the relevant vendor. In the unlikely event that a vendor experiences a security breach, Southwest Trauma Informed Integrated Care ( STIIC) will make reasonable efforts to notify patients.
Iniital Evaluation
All new patients undergo a comprehensive psychiatric evaluation, so that the provider may make a diagnostic assessment and offer treatment recommendations where relevant. After the evaluation, the provider will determine whether it would be appropriate to establish a therapeutic relationship, and the patient may decide whether to seek treatment. In certain cases, the provider may determine that tele-medicine is not an appropriate mode of treatment. While the provider may offer advice on the alternative treatment to be sought, they are not responsible for the patient's decision on whether and how to seek such treatment. If a patient faces an emergency---including a threat of physical harm to themselves or others---they should seek help from the relevant emergency services.
Clinical Services
The services rendered by Southwest Trauma Informed Integrated Care ( STIIC) are provided on an outpatient basis via advance appointment using secure telecommunication software. To ensure continuity of care and appropriate treatment, patients with whom STIIC has an established therapeutic relationship should have a follow-up appointment at least every three months.
CONTROLLED SUBSTANCES
Federal law places restrictions on the prescription of controlled substances via tele-medicine. This includes stimulants (e.g. Adderall and Ritalin), benzodiazepenes (e.g. Xanax and Klonopin), and sleep medications (e.g. Ambien and Lunesta). Southwest Trauma Informed Integrated Care (STIIC) currently doesn’t prescribe controlled substances via telemedicine, but will be willing to consult with your primary for the best plan of care.
PAYMENT OF SERVICES:
Southwest Trauma Informed Integrated Care (STIIC) currently is out of network and accepts some insurances. Please follow url below for further information on the current insurances accepted. For Out of network patients, STICC will give the patient a super bill to submit to their insurance carrier. While patients will on request be provided with documentation with which they may seek reimbursement, STIIC Psychiatry does not bear responsibility for the success or otherwise of such insurance claims. Payment details is essential to be provided during appt booking. This information securely stored by a third party payment processor. By providing these details, the patient consents for STIIC to pre-authorize the payment card in advance of each appointment and to subsequently bill for services at the prevailing fee schedule. In the event of a patient no-show or cancelation that occurs less than 24 hours before the appointment time, or in the event that a patient is not located in the state reported on the intake form, STIIC charges a $50.00 dollar service fee.
